Schedule a Long-Term Care Consultation

Before we meet:
Have a rough picture of your income and assets. Ballpark ranges are fine, but exact current figures are helpful.

Income (monthly or annual):
Salary/consulting, Social Security, pensions
Rental income, dividends/interest, RMDs
Annuity income (if any)

Assets (approximate values):
Checking/savings, brokerage/taxable accounts
Retirement: 401(k), IRA, Roth, 403(b), TSP
Annuities (qualified or non-qualified). Note current value and surrender period, if known
Life insurance cash values (if any)
Real estate (est. equity), business interests

Why I ask: It helps me design funding options (e.g., 1035 exchanges, RMD coordination, premium structure) and match carriers appropriately without wasting your time.

Privacy: Please don’t send statements or account numbers before we talk. You’ll get a secure upload link if we need documents later.
